The journey
Simple from the first interaction.
They answer up to 20 questions: single or multiple choice, a rating from 1 to 5 or 1 to 10, or a short text.
A question can depend on an earlier answer, so each person sees only the questions that apply to them.
The answers reach the console, ready to review and export.

Branching the server checks
Show a question only when an earlier answer calls for it. The server works out which questions apply and refuses a missing answer, or an answer to a question that was skipped.
Anonymous unless they choose
The contact form is optional, so a survey can ask for nothing but the answers. Add a prize for finishing when you want more responses.
Answers you can use
Review the results question by question and export the responses as CSV. Choice answers export without contact details; free-text answers go with them only when the person’s consent allows it.
A post-event survey, a reader survey for a media brand, or a customer feedback round after a purchase.
